Dr. Lauren Chapman
I’m truly passionate about providing compassionate, dignified care to animal companions during their final stage of life.
My love for animals has guided my path for as long as I can remember. While studying Animal Science at The Ohio State University, it became clear I wanted to dedicate my life to animal well-being and to supporting the families who love them. I went on to earn my veterinary degree from The Ohio State College of Veterinary Medicine, and from there, my path led me to focus on end-of-life care.
Specializing in in-home euthanasia lets me blend medical expertise with a heartfelt commitment to creating a peaceful, comfortable environment for animals in their final moments. Every animal deserves a dignified, pain-free transition, surrounded by love, in the comfort of their own home.
At Transitions Veterinary Services, we provide gentle, individualized end-of-life care for each patient, right where they’re most comfortable. We work closely with families to help them understand their companion’s condition, answer their questions, and guide them through this difficult decision, focusing on compassion, comfort, and a peaceful farewell.
I consider it a profound privilege to be welcomed into the lives of these animals and their families during such an emotional time. I’m humbled by the trust placed in me, and I remain committed to meeting it with expertise, empathy, and steady support.
Outside of work, you’ll usually find me outdoors, finding solace and inspiration in nature, a reminder of the same deep bond we share with the animals in our care.

Dr. Samantha Cheng

I’m Dr. Samantha Cheng, and I’m a part-time veterinarian with Peaceful Veterinary Care. When I was an undergraduate at USC (Go Trojans!), I started my first job as a kennel assistant at a general practice clinic in my hometown of Pasadena. I fell in love with veterinary medicine and decided to pursue my veterinary degree at University of California, Davis. Since graduation I have settled in San Diego and work as an Emergency and Critical Care Veterinarian.
Working in emergency medicine has been very gratifying as I’m able to make a big, tangible impact in the lives of both pets and owners in very demanding situations.
However the emergency room is often also a place where pets and their parents have to part ways because a disease or trauma is too devastating to overcome. Oftentimes the decision of pursuing humane euthanasia has to be made quickly without preparation during a highly stressful situation. Helping pet parents make this painstaking decision and allowing pets to pass peacefully and with dignity has been a powerful experience and a newfound passion.
Through Transitions, I have the honor of helping families through this process in a more familiar and peaceful setting. I hope I have the privilege of meeting you and guiding your family through this difficult chapter.
Dr. Lynsey Hillberg
Dr. Lynsey Hillberg grew up in California’s Central Valley and trained as a veterinarian at University College Dublin in Ireland. As a student, she loved exploring the Irish countryside, caring for animals in mixed practice, and enjoying a warm cup of tea between visits.
Now based in San Diego, she practices small animal medicine and provides in-home euthanasia, believing every pet and family deserves a peaceful, compassionate, and dignified transition. She finds it deeply meaningful to support families through this difficult time in the comfort of their own home.
She lives with her tripawd rescue dog, Astro, who keeps her active and blissfully unaware of his missing leg, and enjoys exploring San Diego’s beaches, trails, and all the area has to offer.

Amanda Hodder
Amanda knew from a young age that her love for animals would lead her to do great things. She could always be found with a pet or animal by her side. Her friends have jokingly call her Snow White, because stray animals always seem to find her. In 2017 she followed that passion and started fostering kittens, and the next year she began the process to open her own 501c3 nonprofit kitten rescue. Since then her volunteer-operated organization has saved the lives of hundreds of kittens. In 2020 she became a mother to a little boy named Zephyr, and in 2021 she became part of the Transitions team. On top of running the rescue and working for Transitions she is a stay-at-home mom.
